REQUIREMENTS:

• Member or Retiree of the 180th Fighter Wing

• Military Service of at least five years

• There is a documented need for financial assistance for necessities of daily living that include food, clothing, housing, utilities, medical service, prescriptions, insurance payments, funeral, or other needs.

• The service member must be serving honorably or must have received an honorable discharge.

• The applicant must sign and date the application and provide all required proof and documentation.

• The qualified individual or family member must be a spouse or child living in the household, and can receive up to $2,000.00 one time from the Sonny Smiles Foundation Family Support Fund.

• The Sonny Smiles Foundation will pay expenses directly related to the service member’s hardship.
